Job Content:

Responsibilities:

• Build up effective community networks with external stakeholders
• Facilitate promotion of services and recruitment of clients 
• Develop Primary Healthcare Resources Hub with other team members
• Coordinate District Health Center Express program schedules together with other teammates
• Supervise social work and program team
• Deliver direct professional practice in services and team coaching/training as appropriate on case management

Requirement:

• Degree holder or above in Social Work 
• Registration with the Social Workers Registration Board
• Eight or more aggregated years of practice in social or medical-social field preferably
