Job Content:

Responsibilities: 

• Act as a case manager of disease /complication screening, coordinate referrals to & from other medical and allied health disciplines, provide health coaching and health education at the Core Centre, Service Points and outreaching sites;
• Plan and Conduct health promotion & health prevention programs;
• Engage and liaise with service partners in the community
• Any other primary nursing service initiatives; As active member of multi-disciplinary team

Requirements:

• Degree in Nursing or equivalent; holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong; CC: possesses 6 years or above of clinical experience as registered nurse; RN: possesses 3 years or above of clinical experience as registered nurse
• Post registration experience in Primary Health Care, Women Health Services or Elderly Health Services would be an advantage.
• Good interpersonal, problem-solving, communication and presentation skills 
• Proactive, mature and independent; possess good computer literacy
• Working Location: Tseung Kwan O
